What Type of Information do I collect?
When I receive an enquiry or you book an appointment I will take your name, address, telephone number and email in order to stay in touch and to remind you about your future treatments. I may contact you if I have any special offers or if I haven't heard from you in a while.
During appointments I will also take certain medical history information and information about any allergies you may have. This is to ensure you are safe to have the treatment and to prevent any unwanted reactions. I will also keep a record of your treatment history and any other notes relevant to the treatments.
How do I collect your Information?
When you book an appointment I will ask for any details I require to ensure the treatment is suitable, and any contact information I need. On the day of your (first) treatment you will be required to fill in a consultation form with your contact details and brief medical history. I also collect the information you input on the contact forms on my website.
